Revisions and caveats – December 2025 Release
Immunisations indicators: From 2022/23 onwards, the data has been revised to use population information based on the 2021 Census. This update may lead to differences when compared with figures published previously.

Data Quality:
The issue with clinical coding completeness highlighted in our previous update remains ongoing. Due to continued staffing and resource constraints within the NHS Wales Clinical Coding Service, the number of Admitted Patient Care (APC ds) episodes assigned classification codes in the 2024/25 financial year is significantly lower than in previous years. This reduced coding coverage may affect any all-Wales analysis of coded data. For further information, please contact clinical.coding@wales.nhs.uk.

Admitted Patient Care (APC) Data Quality 2024/25:
Of the seven Health Boards only Powys Teaching Health Board achieved 100% Clinical Coding Completeness for the rolling 12- month period, 2024-25. Hywel Dda University Health Board and Cwm Taf Morgannwg UHB met the 98% target. The remaining health boards – Aneurin Bevan UHB (87%), Cardiff and Vale UHB (81%), Betsi Cadwaladr UHB (79%) and Swansea Bay UHB (79%) – were more than 4% below the target. **23 October 2025 – Please note that the data originally published in the March 2025 release for the indicator ‘Osteoporosis, Admission Rates (Age-standardised) per 100,000 population, Persons, All Ages, 3-Year Average’ contained inaccuracies. These have now been corrected. We apologise for any inconvenience this may have caused.

Data Quality:

Only Powys Teaching Health Board (100%) and Cwm Taf Morgannwg University Health Board (98.4%) achieved the 98% Clinical Coding Completeness target for rolling 12 months’ data. Hywel Dda University Health Board (96.9%) was 1.1% off the target. The remaining health boards, Aneurin Bevan ULHB (82%), Betsi Cadwaladr ULHB (76.1%), Cardiff & Vale ULHB (75.4%), and Swansea Bay ULHB (84.8%) were more than 4% below the target. The Welsh Index of Multiple Deprivation (WIMD) data recorded on Health Maps Wales was been incorrectly published as 2019 data when the figures published were for 2014. The WIMD indicators have now been updated with the correct information.

The Welsh Index of Multiple Deprivation (WIMD) data recorded on Health Maps Wales has been incorrectly published as 2019 data when the figures published were for 2014. The WIMD indicator has been removed from Health Maps Wales until we are able to resolve this. We apologise for any inconvenience caused.

• Please note that the latest Health Maps Wales publication for Wales, Local Health Boards and Local Authorities uses the rebased ONS population estimates following the 2021 Census. This has resulted in changes to the population estimates for 2012 – 2021. Indicators for USOA, MSOA and LSOA geographies remain based on 2011 Census population estimates. ONS are currently conducting reconciliation work which will result in a revised back series of population estimates for these Output Areas. Indicators for these geographies will be updated once ONS have released this information. Please see www.ons.gov.uk for further information.

Hywel Dda ULHB (95%) was within 4% of the target and the remaining health boards, Aneurin Bevan ULHB (79.5%), Betsi Cadwaladr ULHB (91.9%), Cardiff & Vale ULHB (72.8%), Cwm Taf Morgannwg ULHB (78.8%), and Swansea Bay ULHB (83.8%) were more than 4% below the target. March 2023

• Please note that the 2021 data is based on the Census 2021 population estimates released by ONS. ONS are currently conducting reconciliation work which will result in a revised back series of population estimates for the period 2012 to 2020 to be released later in 2023. Care should be taken when comparing rates from 2021 with previous years until the back series has been revised.

• Admitted Patient Care (APC) Data Quality 2021/22 – All Health Boards failed to achieve the 98% Clinical Coding Completeness target for rolling 12 months’ data. Five organisations Aneurin Bevan (86.4%), Cardiff & Vale (85.9%), Cwm Taf (85.6%), Hywel Dda (92.1%) and Swansea (89.3%) were more than 4% below the 98% target. • Outpatients – In May 2022, Powys resubmitted data for 2018/19 onwards to include their Nurse Led clinics activity therefore rates may not reflect those published previously.

• ONS Births data – Please note that ONS resubmitted their births data in October 2022 to include 2020 late registrations therefore rates may not reflect those previously published.

• NCCHD Births data – Please note that our previously published rates were based on only the most recent years population estimates. These have now been revised to be calculated on the population estimates provided by ONS for each year. Please note that these will be subject to change again when ONS release the revised population estimates for the period 2012 to 2020.
